If wanting to use insurance, please also list the insurance company name and your member ID #, if available, so we may confirm your member benefits and out of pocket costs prior to returning your email.

We offer a brief 10 minute video or phone consultation and please specify this in your message, but it is recommended to make the decision if you would like to work with us based on the assessment and treatment recommendations you and your provider will discuss in the initial intake and evaluation appointment. The treatment is based on you, and we need to get to engage in a full evaluation prior to being fully equipped to discuss the most optimal treatment options.
